Ultra Comprehensive Physical Therapy & Occupational Therapy is a healthcare practice offering a range of therapy services to individuals in need of physical rehabilitation. These services include but are not limited to Physical Therapy, Occupational Therapy, Speech Therapy, Surgery Rehabilitation, Injury Assessment, Sports Medicine, Manual Therapy, Home Care Services, and Transportation Assistance. Our clinics are located in Clifton, NJ, and Maplewood NJ.

 2. Use of Services

To use our services, you must:

  • Eligibility: Be of legal age (18 or older) or have parental or guardian consent if underage.
  • Accurate Information: Provide truthful and complete details when scheduling appointments or undergoing treatment.
  • Compliance: Follow the treatment protocols and instructions provided by our licensed healthcare professionals to ensure the best possible care.

We reserve the right to deny, suspend, or terminate services if you fail to adhere to these Terms.

 3. Appointment Scheduling and Cancellations

  • Booking Appointments: You may schedule appointments via phone or our online system. To ensure timely care, please confirm your appointments at least 24 hours in advance.
  • Cancellations: We require at least 24 hours’ notice for cancellations to avoid any cancellation fees. In the absence of proper notice, a missed appointment fee may apply.
  • Late Arrivals: Arriving late may reduce the length of your treatment session or require rescheduling, depending on the clinic’s schedule.
  • Transportation Services: For patients requiring transportation to and from appointments, please arrange this in advance. Ultra Comprehensive Physical Therapy & Occupational Therapy is not responsible for delays caused by third-party transportation providers.

 4. Payment and Billing Policies

  • Payment Responsibility: You are responsible for ensuring payment for all services rendered at the time of your visit or per your agreed-upon payment plan.
  • Insurance: We accept most major insurance providers. It is your responsibility to verify with your insurer that we are within your network and to confirm the coverage for the services we offer.
  • Out-of-Pocket Expenses: If your insurance does not cover certain services or if you are self-paying, you are responsible for all costs incurred.
  • Late Payments: Payments not received by the specified due date will incur late fees, and any unpaid balances may result in suspension of services until payment is made in full.

 5. Privacy and Confidentiality

We value your privacy and are committed to safeguarding your personal and health-related information. Our Privacy Policy outlines how we collect, use, and protect your data.  6. Limitation of Liability

Our liability is limited to the cost of the service that was provided. We will not be liable for any indirect, incidental, or consequential damages resulting from your treatment.

 7. Termination of Services

We reserve the right to suspend or discontinue services to any patient who:

  • Violates these Terms and Conditions.
  • Fails to comply with treatment recommendations or protocols.
  • Provides false or incomplete information.
  • Engages in disruptive, inappropriate, or unsafe behavior.

In the event that services are terminated, any outstanding balances will remain due.

 8. Modification of Terms

Ultra Comprehensive Physical Therapy & Occupational Therapy reserves the right to modify these Terms and Conditions at any time without prior notice. Changes will be posted on our website, and the updated Terms will be effective immediately upon posting. It is your responsibility to review these Terms regularly.

By continuing to use our services after modifications have been made, you accept the updated Terms and Conditions.

In case of any dispute regarding these Terms and Conditions or the services we provide, we encourage you to contact us directly to resolve the issue. If the matter cannot be resolved informally, any legal action or dispute will be subject to the laws of the state of New Jersey and will be adjudicated in the appropriate courts located within New Jersey.
